Varicocele also called pampiniform plexus of veins means abnormal distended veins in the scrotum. The veins get enlarged within the scrotum( a skin-covered pouch that holds testicles). The scrotum contains a set of veins that deliver blood to the reproductive system. When these veins turn abnormal(enlarged veins) then varicocele occurs. Varicocele is closely related to varicose veins. A varicocele reduces the production of sperm count and quality, and in some cases shrinks the testicles, causes infertility, and affects the reproductive system too. This situation is commonly faced by men between 15 to 25 age. These veins may exist on both the sides of scrotum causing potential discomforts.
Well, varicocele is mostly identified when you notice some changes in your body like-
Valsalva maneuver, physical examination, and scrotal ultrasound are a few ways used to diagnose varicocele. The grade and severity of varicocele are discussed by health professionals soon after diagnosing. Further treatment is followed depending upon the degree of the issues experienced by the patient.
Varicocele treatment typically involves two main approaches: conservative management and surgical intervention. The choice of treatment depends on factors such as the severity of the varicocele, symptoms, and the individual’s overall health
Conservative Management: Mild varicoceles that don’t cause significant discomfort or fertility issues may be managed conservatively. This includes lifestyle changes such as wearing supportive underwear, avoiding prolonged periods of standing, and using over-the-counter pain relievers if needed.
Surgical Intervention: If the varicocele causes pain, discomfort, or affects fertility, surgical treatment might be recommended. There are different surgical techniques, including:
Varicocele embolization is a minimally invasive medical procedure used to treat varicoceles, which are enlarged veins in the scrotum that can cause discomfort and potentially affect fertility. Here’s an overview of the procedure:
Before embarking on treatment, a thorough evaluation is conducted, often including a physical examination and imaging tests like ultrasound to confirm the presence and severity of the varicocele.
Before the procedure, the patient will undergo some tests, such as an ultrasound, to confirm the diagnosis and assess the extent of the varicocele. On the day of the procedure, the patient is typically advised not to eat or drink for a certain period of time prior to the treatment.
Local anesthesia is administered to numb the area where the catheter will be inserted, usually in the groin.
A thin, flexible catheter is inserted through a small incision or needle puncture, guided by imaging techniques such as fluoroscopy (real-time X-ray) or ultrasound.
The catheter is carefully threaded through the blood vessels until it reaches the affected varicocele.
Once the catheter is in position, a contrast dye may be injected to enhance the visibility of the blood vessels on X-ray or fluoroscopy. Small coils or a solution (sclerosing agent) are then placed within the varicocele. This blocks the blood flow and causes the vein walls to stick together.
Once the embolization is complete, the catheter is removed, and the incision site is usually closed with a small bandage.
Most patients can go home on the same day or shortly after the procedure. Some discomfort, bruising, or swelling at the catheter insertion site is normal and should subside within a few days.
Over time, the body naturally absorbs the blocked vein, rerouting blood flow to healthier veins. This helps relieve symptoms and improves blood circulation in the scrotum, potentially addressing pain and fertility issues.
Patients may be advised to rest and avoid strenuous activities for a few days after the procedure. A follow-up appointment is scheduled to monitor progress and address any concerns.
Varicocele embolization offers several benefits for individuals experiencing varicoceles.
Percutaneous embolization or varicocele embolization is generally considered a safe and effective option for treating varicoceles with a shorter recovery time compared to traditional surgery. As with any medical intervention, it’s important to discuss potential risks, benefits, and alternatives with a healthcare professional before proceeding.
